Please complete the online questionnaire. At the end of the questionnaire there is a tab to submit the form direct to the hospital. The referral is then processed by the hospital and you will be invited for an appointment.It is recommended that you self-refer prior to nine weeks, although this is not always possible, and can be done even if past nine weeks.
