A PCN is a group of practices that have chosen to work together with various other local providers across primary care, community services, social care and the voluntary sector. The aim is to provide more personalised, coordinated health and social care to the local population, whilst also supporting local practices to be able to share resources.
Cheam GP Centre is part of Cheam and South Sutton PCN (CASS). The other practices within this network are Benhill and Belmont Surgery, Cheam Family Practice and James O’Riordan Medical Centre.
